PROCESS

1. 1. Student picks up a carbon copy PE Uniform Purchase Form (see picture below) from the main office.
   

2. 2.Form is completed by Student and/or Parent along with cash, check or money order.  Please indicate correct size on this form.
   

3. 3.Form and money are turned into the Main Office
   

4. 4.Main Office issues a copy of the PE Uniform Purchase Form to the Student and the Student then gives it to a PE Teacher in their respective locker room
   

5. 5.Appropriate type, size and quantity of Uniform(s) are issued to Student.
   

6. 6.Student needs to make sure that their name is written in permanent ink on both the shirt and shorts.  If time, a teacher can do it before student uses them.
